Evaluación del monitoreo de anticonvulsivantes en un centro de referencia para el tratamiento de la epilepsia en Bogotá

Therapeutic monitoring evaluation of antiepileptic drugs in a reference center for the treatment of the epilepsy in Bogotá

Se diseñó un estudio observacional, descriptivo y retrospectivo para evaluar el monitoreo de anticonvulsivantes mediante el análisis de 1017 consultas correspondientes a 272 pacientes que ingresaron por primera vez a la institución entre agosto de 1998 y agosto del 2000. Los parámetros analizados fueron los establecidos por la Liga Internacional contra la Epilepsia (1993) para el monitoreo de anticonvulsivantes. Se solicitó nivel sérico en el 32.1% de las consultas que lo requerían y se les practicó la determinación al 11,2% de estas. Ninguno de los reportes de nivel sérico contiene información referente a régimen terapéutico, hora de toma de la última dosis ni hora de toma de la muestra. Se detectó un bajo cumplimiento de los lineamientos establecidos para la solicitud de niveles séricos. La ausencia de información en cuanto a las condiciones de toma de la muestra de sangre hace que los datos obtenidos pierdan utilidad y no se puedan realizar con ellos los cálculos farmacocinéticos necesarios

In order to evaluate the anticonvulsant monitoring in a Colombian reference center, an observational, descriptive, and retrospective study was made analyzing 1017 consultations of 272 patients who entered by the first time in the institution during August 1998 to August 2000. The Guidelines for Therapeutic Monitoring on Antiepileptic Drugs elaborated by the Commission on Antiepileptic Drugs of the International League against Epilepsy were the parameters used for comparison. The blood levels were prescribed in 32.1%, of the consultations that need them. The patients’ compliance was 11.2%. The blood level reports do not have information about the therapeutic regimen, timing of the last dosage, and timing of the blood sample. In the reference center there is not have an appropriate compliance of the therapeutic drug monitoring. The information obtained is not enough and the pharmacokinetic estimates can not be made. It is noted the absence of Pharmacist staff in this process.
