V. Cordiere, Profesor Agregado de la Facultad de Lyon, define el hipo: Sacudida brusca de los músculos respiratorios y abdominales, con ligera eructación y ruido laríngeo súbito.
Emile Sergent,quien no le da gran valor semiológico, dice que es “una inspiración brusca repetida un gran número de veces al minuto y acompañada de un gran ruido inspiratorio".
Mecanismo: Es el hipo una contractura tónica brusca del diafragma y de los músculos intercostales a la cual se asocian los músculos constrictores de la glotis; el nervio frénico es el causante.
A la radioscopia, esta contracción súbita del diafragma es unilateral en los sujetos que han sufrido la frenicectomía. Se puede provocar el hipo por excitación del frénico. Muchas son las causas del hipo: Puede aparecer durante el periodo digestivo aun en individuos sanos y se observa en los sujetos neurópatas durante las comidas o algún tiempo después de ellas, a consecuencia de una emoción, de una contrariedad o sin causa alguna.
Causas gástricas: Si el estomago está lleno de gases o después de la ingestión de alcohol en un individuo no habituado a su uso, el hipo es frecuente; también se le encuentra en la ptosis gástrica, Es raro en el curso de afecciones graves como el cáncer gástrico, la ulcera del estomago o la hiperclorhidria.
Causas peritoneales: Se encuentra este signa gástrico en el curso de las peritonitis agudas, por perforación o por apendicitis; en estos casos revela una lesión profunda del peritoneo y a menudo es un síntoma terminal.
