Patología de la infección por Helicobacter pylori

A partir de la descripción inicial del Helicobacter pylori (inicialmente denominado Campylobacter pylaridis y posteriormente C pylori) por Warren y Marshall en 1983, han comenzado a dilucidarse gran cantidad de interrogantes sobre aspectos diversos de la etiopatogenia de un significativo número de enfermedades gastroduodenales. Una oleada creciente de investigaciones realizadas, particularmente a partir de 1986, permitió que el escepticismo sobre el papel etiológico de este nuevo agente empezara a ser reemplazado par el interés en profundizar el conocimiento sobre su microbiología y mecanismos patogénicos, su interrelación con el huésped, su asociación con enfermedades especificas, su impacto epidemiológico y la respuesta de los pacientes a terapia específica contra el H pylori. Para ella se han utilizado metodologías diversas en los campos de la gastroenterología, la patología, la microbiología, la inmunología etc., e inclusive se han desarrollado técnicas novedosas e ingeniosas como el test respiratorio para detectar la presencia de infección activa sin recurrir a técnicas invasivas. Dentro de este proceso, la patología ha jugado un papel fundamental, inicialmente identificando la bacteria en muestras de biopsias y especímenes quirúrgicos definiendo la asociación causal entre lesiones de la mucosa gástrica bien reconocidas des de tiempo atrás en 32 diversas entidades nosológicas y el H pylori, contribuyendo de esta manera a redefinir su patogenia, a evaluar la respuesta a los tratamientos de erradicación.
