Experiencia con 1000 implantes subdérmicos de levonorgestrel en el Instituto Materno Infantil (I.M.I).

Colombia ha incrementado el uso de métodos modernos como los implantes subdérmicos de levonorgestrel (Nor-plant), en los programas de Planificación Familiar. El IMI inició sus experiencias con ellos en 1984, perú solo por 2 años. Desde 1992 en un programa con la OPS se reinicia su aplicación y al llegar a 1000 implantes, queremos mostrar esta experiencia. Objetivos: evaluar eficacia, aceptabilidad, tolerancia y efectos secundarios del Norplant. Materiales y Métodos: primeras 1000 pacientes del IMI con Norplant en los últimos 5 años. Resultados: la mayoría de usuarias tenían entre 16-33 años y de 1-3hijos. La eficacia hasta ahora es de 98.6%. La mitad de las fallas fueron gestaciones ectópicas. No hubo variaciones importantes de peso. Los efectos secundarios mas frecuentes fueron los trastornos del ciclo y cefalea en el primer año de uso. E117% de los implantes se han retirado. E191.9% de las pacientes manifestaron satisfacción con el método y la continuidad para el primer año fue de 94% y para el 4 año de 78%.
Conclusiones: el implante subdérmico de levonorgestrel, es eficaz, aceptable y seguro. Las molestias inmediatas a la inserción son mínimas y los efectos secundarios deben ser conocidos por la paciente en el proceso de consejería. Se requiere mejor conocimiento del método por parte de los médicos generales, para orientar mejor a las pacientes.
Colombian Family planning programs have increased modern methods like Norplant. The Instituto Materno Infantil, had Norplantprogram from1984 to 1986 but in 1992 with the support of the PHO began a new program. Now we have 1000 patients with Norplant. Objective: To evaluate Norplant efficiency, tolerance, acceptability and side effects. Results: Most of patients had between 16-33 years old and had between 1-3 children. Norplant efficiency was 98.6%. There were seven ectopic pregnancy (50% of failure).
The main side effects were irregular menstrual bleeding and headache. 17% of implants have been removed. 91.9% were satisfied and continuity rate was 94% in the first year and 78% at the end of the fourth year. Conclusion: Norplant is a safe and effective hormonal method. Counseling is necessary before Norplant use. It is necessary to improve Not-plant knowledge in physicians in order to offer better medical counseling.
