Phimosis in a cat: diagnosis, treatment and complications

Phimosis is the inability to advance the penis through the preputial orifice. This condition in its congenital form is infrequent in cats and has a low prevalence when compared to the canine condition. Diagnosis is based on the physical exam and the treatment of choice is often preputioplasty. The following case describes a six-week-old feline patient with signs of strangury, pollakiuria, excessive licking, and vocalizations when urinating. The physical exam encountered a significant stenosis of the preputial orifice. A circular preputioplasty was the therapy performed and three days later the patient presented with recurrent signs. Excessive granulation tissue was observed around the surgical wound, prompting further corrective surgery. This time the patient showed immediate resolution of the signs of distress that after 1 year have not recurred. The objective of this study is to contribute information about clinical-surgical decisions and the principle complications presented by cats with phimosis.
La fimosis es la incapacidad para exponer el pene más allá del orificio prepucial. La fimosis congénita en gatos es poco común y presenta baja prevalencia en comparación con la presentación en perros. El diagnóstico se basa en el examen físico y el tratamiento de elección es la prepucioplastia. Se describe el caso de un paciente felino de 6 semanas de edad con signos de estranguria, polaquiuria, lamido excesivo y vocalización al realizar la micción. En el examen físico se aprecia una marcada estenosis del orificio prepucial. Como abordaje terapéutico se realiza una prepucioplastia circular y al cabo de 3 días el paciente se presenta con una recidiva de los signos iniciales. Se observa excesivo tejido de granulación, por lo cual se somete a cirugía correctiva. El paciente muestra resolución de los signos inmediatamente después de la cirugía y luego de un seguimiento durante 1 año no vuelve a presentar signos de recidiva. El objetivo de este reporte es contribuir con más información en cuanto a las decisiones clínicas-quirúrgicas y las principales complicaciones en gatos con fimosis.
