Se presenta un caso de dilatación aguda, necrosis y ruptura del estómago, tratado con éxito. Se discute su posible etiología y se evidencia el Clostridium welchii al microscopio. No se efectuaron hemocultivos. El tratamiento inicial fue la succión gástrica por 48 horas con mejoría, pero a las 12 horas de suspenderla se presentó un cuadro agudo con perforación y necrosis, y fue necesario practicarle una gastrectomía total. El paciente presentó como complicaciones post-operatorias una necrosis tubular en el riñón, una parotiditis que se abscedó posteriormente e intoxicación por potasio, y salió finalmente en buenas condiciones del Hospital a los 40 días de su ingreso.
A case of sharp dilatation, necrosis and a rupture of the stomach is presented with a successful treatment. The possibilities of etiologies are discussed and evidence is given of Clostridium welchii through the microscope. No hemocultivation were effectuated. 3. The initial treatment was gastric pumping for 48 hours, with improvement. But, after 12 hours of suspending it, a sharp picture denoting perforation and necrosis was presented, and it was found necessary to perform a total gastrotomy. The patient later had post operative complications of necrosis, a tubular in the kidneys, a paratiditis, and finally an intoxication of potassium, which formed an abscess. After 40 days of hospitalization, he was released and in good health.
