Un caso de infeccion humana por T. rangeli

Se presenta el estudio clínico y experimental de un enfermo que
fue observado por los autores durante 16 meses. El cuadro clínico estaba caracterizado por fiebre prolongada (65 días en una de sus fases), hepato-esplenomegalia, anemia, edemás maleolares fugaces, dolores osteomusculares intensos, insomnio, leucocitosis con linfocitosis, eritrosedimentación acelerada. De la sangre del enfermo se aisló un trypanosoma cuyos caracteres morfológicos lo diferencian claramente del T. cruzi y coinciden con los del T. rangeli Tejera. El estudio experimental del flagelado se llevó a cabo sobre material obtenido de hemocultivos, xenodiagnósticos con R. prolixus
y T. capitata, e inoculaciones. Como novedad en estos métodos se recomienda la adición de penicilina o tirotricina al medio de Geiman para cultivos con el fin de obtener un mínimo de contaminaciones. Finalmente, se discuten las relaciones del trypanosoma aislado con la sintomatología del enfermo y la acción terapéutica de la Cloroquina (W7618), que produjo la curación clínica. La negativización de los cultivos sólo se consiguó después de aplicar antimoniales.
