Given the diverse roles that internists fulfill in providing comprehensive adult care across clinical settings (primary care, inpatient consultation, hospitalization, and emergency care)—spanning the prevention, diagnosis, and management of complex chronic and acute conditions, as well as timely referral to other specialists or subspecialists— these professionals are critical to proper health system functioning. Accordingly, this reflection article aimed to provide an overview of the supply and demand of internists in Colombia, encompassing training capacity and geographic distribution in the country.

Evidence retrieved (comprising official records from the Observatorio de Talento Humano en Salud del Sistema Integrado de Información de la Protección Social [Health Human Resources Observatory module of the Integrated Social Protection Information System, SISPRO by its Spanish acronym], five studies, and two official Ministry of Health documents) indicates that the national supply of internists is not notably low compared to other countries. In fact, this supply has progressively increased over the past 15 years (2011-2025), rising from 0.5-0.95 (varying by source) to 1.31 internists per 10 000 inhabitants in 2025, which has been largely driven by more training capacity (39 active residency programs in the first semester of 2026), substantially narrowing the supply-demand gap.

If this trend persists, the national demand for internists is projected to be fully met within the next 2 to 3 years. However, the primary challenge lies in the marked geographic maldistribution of these specialists across the country’s 32 administrative departments, a persistent issue over the last 15 years characterized by heavy concentration in 4 or 5 departments (66.3% of internists were practicing in just 4 departments). Consequently, moving forward, policy efforts must focus on improving the availability of and access to these specialists in regions reporting rates below 1 internist per 10 000 inhabitants.

Lastly, official reports and published literature regarding the supply and demand of internal medicine workforce in Colombia remain scarce and outdated. Discrepancies were also identified between published estimates and the active internist workforce figures reported in the basic indicators of human resources in health from SISPRO for the 2011-2025 period.
