La oncología ha llamado de regreso a la medicina mecanicista y a la causalidad determinista para orientar su quehacer investigativo en la terapia antineoplásica. A lo que ha llevado esta postura epistemológica, es a un cambio regresivo del programa de investigación terapéutica en cáncer, manifiesto por las modificaciones introducidas en los desenlaces de los ensayos clínicos con agentes citostáticos ante el fracaso del desenlace esperado de regresión tumoral y curación. Ya en estudios fase II con agentes citostáticos no se habla de tasas de remisión tumoral sino de mediciones subrogadas, muchas aún no validadas, del efecto citostático sobre el marcador molecular, y en los desenlaces de estudios fase III se incluye como criterio de respuesta la estabilización o no progresión tumoral. Todo un fracaso del prometido y anhelado “proyectil mágico” de la terapia orientada a blancos moleculares en cáncer.
Oncology has called the return of the mechanistic medicine and deterministic causality to orient investigative tasks in cancer therapy. Due to failure in cancer regression and healing as expected end points in clinical trials with cytostatic, researchers had introduced these epistemology modifications. Already ongoing studies in phase II with cytostatic agents don´t talked of rates of tumor remission but the do of subrogated measurements, many not yet validated, like the effect of cytostatic on molecular scoreboard. In the phase III ongoing studies these results are included like criterion of answer instead of failure cancer progression or healing. A true disappointment of cancer molecular therapy.
